Cancellations: please email info@hustle-hoops.com Liability Policy: Informed Consent and Acknowledgment I, the undersigned, acknowledge that participation in basketball activities involves inherent risks of injury. I understand that these risks include, but are not limited to, physical injury, psychological stress, and damage to personal property. Upon purchase of any service, I accept full responsibility for these risks. Release of Liability In consideration of being permitted to participate in basketball activities at a Hustle Hoops training location, I agree to release and hold harmless any location or establishment, its owners, employees, agents, and volunteers from any and all liability, claims, demands, or actions arising from or related to any injury, loss, or damage incurred during my participation, whether caused by negligence or otherwise. Assumption of Risk I understand that basketball is a vigorous sport and can result in injury. I acknowledge that I, or the person I am registering, am in good health and have no known medical conditions that would contraindicate my participation. I agree to follow all gym rules and instructions to minimize risks. Medical Consent In the event of an emergency, I consent to medical treatment deemed necessary by a qualified medical professional. I understand that I am responsible for any costs associated with such treatment. Acknowledgment of Understanding I have read and understand this waiver, and I am aware that upon my purchase of service(s), I am giving up substantial rights, including my right to sue. I sign this waiver voluntarily and with full knowledge of its significance.
